Covid-19 Clinical Contact Caseworker

Use your clinical skills and expertise in order to help support the NHS – and all from your own home. By working as a Clinical Contact Caseworker, you will be responsible for liaising over the phone with Covid-19 patients, understanding their situation and assisting in the tracing and tracking of anyone they have been in contact with. This crucial role has been developed as part of the Public Health England Contact Tracing Service programme and will allow you to further play your part in defeating the pandemic. This initiative is supported by Department of Health & Social CareNHS England & NHS Improvement and the Cabinet Office.

Please note: the skills and experience required for this position are for those who are currently registered with an appropriate health or science related professional body and are working, or have worked at a Band 6 level. However we would also welcome applications from clinicians above this band who want to play their part on the virtual frontline. Key features and responsibilities of this role include:

  • Undertaking the initial interview of COVID-19 positive cases by phone

  • Conducting a public health risk assessment

  • Identifying contacts and providing public health advice (where appropriate)

  • Working within recognised procedures, scripts and structures, with support

Along with your public health experience, you will have the ability to work independently, problem solve, manage and prioritise your own workload and cope with competing demands.

As part of this role, you will be provided with appropriate training, onboarding and induction materials. Working as part of a wider team of experienced clinicians, you will be liaising with initial call handlers and escalating any in-depth queries to the Public Health England team.

This position also requires excellent communication skills, both verbal and written, including a good telephone manner and the ability to remain calm under pressure. Due to the homeworking and online nature of this role, you must be proficient in the use of Microsoft products, electronic systems and databases.

Advanced Nurse Practitioner

We have a vacancy for a 3 day a week Advanced Nurse Practitioner to work in our Same Day Team, ideally working Monday, Wednesday and Friday each week from 9am to 6pm. The role is to triage patients who have made contact via digital means or called in to seek advice from a healthcare professional. You will need to prioritise care and direct to the most appropriate clinician; this will involve telephone and/or video consultations and diagnosing, treating and prescribing for patients within the boundaries of competence and recognising where support may be needed. Where appropriate, patients may also need to be invited to the Surgery for a face to face consultation for you to safely diagnose, treat and prescribe within the boundaries of your competence. Follow up appointments may be arranged where deemed appropriate and safe; these could be via telephone, video or face to face.

You will be fully supported by our Same Day Team Lead who is a very experienced Advanced Nurse Practitioner along with a Duty GP assigned from the GP Partners and Salaried GPs on a rota basis. We intend to bring in another part time Health Professional to join the team as Primary Care regains full functionality.  We also have Patient Pathways Leads to support the Same Day Team as well as work with eConsults, 2 week waits, referrals and tasks and we have upskilled our Patient Services Team to assist with workflow and documentation management. There is also a Prescribing team led by a Senior Pharmacy Technician and we have a Pharmacist supporting us on a part-time basis from our Primary Care Network.

We are looking for an Advanced Nurse Practitioner who may be either recently qualified or experienced, although we are looking for someone who has at least 1years experience within Primary Care or a ‘first presentation’ setting in some capacity and preferably also has experience of telephone triage work. You do need to be compassionate, resilient, calm under pressure, adept at problem solving, have excellent communication skills and have a teamwork mentality with a good sense of humour. Come and join the Albany urgent care team! We are looking for an Advanced Nurse Practitioner or Emergency Care Provider to work 37.5 hours a week, with some flexibility to start and finish times possible. We are a town practice with a semi-rural catchment area, a growing list size of around 10,500 patients and a strong focus on education, training GPs and medical students. We have an established urgent care team, who work together in a triage hub to help foster case discussion and consistent on-the go support. Our team includes an experienced Advanced Nurse Practitioner, a trainee advanced nurse practitioner and GPs and covers our acute and minor illness ‘on-the-day work’, including home visits.

You will be triaging patients and prioritising their care; as part of your triage you will undertake a mix of telephone calls, video consultations, and home visits, diagnosing, treating and prescribing for patients within the boundaries of your competencies. You will be able to recognise, when you may need support and seek this from an appropriate clinician. Where appropriate, patients may need to be invited to the Surgery for face to face consultations for you to safely diagnose, treat and prescribe. Follow up appointments may be arranged where deemed appropriate and safe; these could be via telephone, video or face to face. Along with the rest of the urgent care team, you will also be expected to carry out some home visits and help process urgent blood results, swabs, urine cultures and documents, with GP support. You will be fully supported by our Urgent Care Team Lead, who is a very experienced Advanced Nurse Practitioner, along with a Duty GP assigned from the GP Partners and Salaried GPs on a rota basis. We have a strong ethos of training and you will be provided with regular mentoring and encouraged to continue to enhance your professional skills. It’s an exciting time to join us, we currently have a Social Prescribing Lead within the team and we have established a need to expand and support the Social Prescribing Role and purpose. We are looking to recruit up to two Full Time Social Prescribers to co-locate within our practices.  We are looking for enthusiastic and motivated individuals to come into our social prescribing team and deliver personalised care to our patient population. You will work as a team member of the Newton West Primary Care Network (PCN), sharing best practice and knowledge for the benefit of patients.

You will be working in partnership with local voluntary, community services and agencies, ensuring the best outcome for patients.

The successful candidates will be working with the PCN under direct supervisions of our Social Prescribing Lead, supporting the three member practices by supporting patients who have long-term health conditions (including support for mental health), people who are lonely or isolated, or who have complex social needs which affect their wellbeing in accessing the services most suited to their needs.

 The role will be pivotal in identifying potential unmet needs in our patient population and addressing inequalities.

'We have an exciting opportunity for a Practice Nurse to come and join our friendly Albany Surgery team. Albany Surgery is in the heart of the Market Town in Newton Abbot. We are looking for a Practice Nurse, who will complement our amazing team of GP Partners, Salaried GPs, Nurse Practitioners, Practice Nurses, Health Care Assistants and Phlebotomist. The ideal candidate will have an excellent skill mix, be friendly, have a good sense of humour and be flexible. We are seeking someone who is enthusiastic about delivering excellent patient care, who possess good interpersonal/communication skills and has the ability to work well independently as well as part of the integrated team. Ideally we are looking for someone to work approximately 20 hours per week spread over 3 days Tuesday, Thursday and Friday (but will need to include Friday afternoons with an 18:00 finish time).
