The Nurse Manager at the Pembroke Medical Group leads a group of 12 Practice Nurses, 1 Assistant Practitioner and 7 Health Care Assistants. These are supported by 2 Nurse Admin Assistants. Although a large team they all work closely together and support each other.

We provide Nurse Cover on 2 sites within Torbay and are part of a wider PCN of 3 Practices. We also have the Extended Access based at our Paignton site and host other services including Lower Limb Service, Midwives, Social Prescribers and Pharmacists.

We support student nurses within the surgery working closely with the University of Plymouth. A preceptorship is offered via the local training hub and we currently have 3 nurses at different stages on this.

The nurse team are looking for someone who will fit in to our team with a good sense of humour, self-motivation and a team player.

We are looking for a committed, enthusiastic and self-motivated individual to work as part of a clinical team with GPs, Advanced Mental Health Practitioners, Practice Nurses, Paramedics and Healthcare Assistants across the PCN.

Torquay PCN is made up of four practices in Torquay – Chelston Hall Surgery, Croft Hall, Brunel Medical Practice and Southover Medical Practice. Together we support a patient list size of around 40,000 people.

As a Paramedic, you will provide a quality service to patients within the PCN, diagnosing and prescribing treatments and medication whilst working within defined procedures and protocols.

You will be responsible for visiting patients with a range of urgent care, minor illness and same day requests for treatment and chronic disease management, clinical assessment, diagnosis, treatment and evaluation of care. This will include some involvement in client home visiting services.

Are you a qualified Dietitian looking for your next role? Are you passionate about supporting patients with complex needs?

An exciting opportunity has arisen for a Dietitian to join our growing Torquay Primary Care Network (PCN) team.

As a Dietitian you will deliver a new model of Dietetic care to adult primary care patients across the Torquay Primary Care Network (PCN).  These adults have multiple long-term, complex conditions and are at risk of malnutrition or are malnourished. 

You will work as a highly autonomous practitioner, often acting as a first contact practitioner within a primary care multi-professional team.

Proactively identifying, assessing and treating a specialist caseload in primary care, care homes and community settings, you will fully integrate within the PCN team and become the source of expert Dietetic support, with the focus on malnutrition.

You will be a highly skilled Dietitian with a dynamic, self-motivated and innovative approach to your work. You will be passionate about patient care, with excellent communication and engagement skills, team working and strong time management. A proportion of your time will be spent in rural areas and you must be able to travel independently and have access to a car on a daily basis.

You will provide training to the wider multidisciplinary primary care team and work alongside the existing Community Clinical Team of Dietitians.

Your role is a key part of our growing and progressive PCN, working to provide an enhanced care experience with a view to improving health inequalities.
