Practice Nurse

We have an exciting opportunity for a Practice Nurse to come and join our friendly Albany Surgery team. Albany Surgery is in the heart of the Market Town in Newton Abbot. We are looking for a Practice Nurse, who will complement our amazing team of GP Partners, Salaried GPs, Nurse Practitioners, Practice Nurses, Health Care Assistants and Phlebotomist. The ideal candidate will have an excellent skill mix, be friendly, have a good sense of humour and be flexible. We are seeking someone who is enthusiastic about delivering excellent patient care, who possess good interpersonal/communication skills and has the ability to work well independently as well as part of the integrated team. Ideally we are looking for someone to work approximately 34 - 37.5 hours over the working week (but will need to include Friday afternoons with a 18:00 finish time). The successful candidate will provide the full range of practice nursing duties including chronic management and diabetes. Salary DOE.

The purpose of the post is delivery of safe, high quality primary care nursing services. Phlebotomist - Part-time

We are currently looking to recruit a phlebotomist for 20 hours per week over 2.5 days to join our busy Nursing team. Working independently, the Phlebotomist is responsible for obtaining and processing blood samples in line with practice policy and procedures. The post holder will also be responsible for processing various samples and recording blood pressure measurements.

'The phlebotomist role involves interacting with a significant amount of patients within restricted timescales. It is important that the post holder is organised and able to work efficiently under pressure. We are a training practice that provides care to a population of 15,000 patients with 6 GP Partners, 7 salaried GPs, 1 GP trainee and a dedicated Nurse team. The Practice team works well together, recognising that a supportive working environment is central to success for individuals and for the Practice.

Practice Nurse 16 hours per week

We are looking for an enthusiastic, reliable and preferably experienced Practice Nurse for 16 hours per week, to join our friendly nursing team of 2 nurses and 2 phlebotomist.  We are a small practice with 5 GPs, based in the beautiful town of Exmouth Devon.

The role will encompass Chronic Disease Management – COPD, Asthma, Diabetes, CHD, Cytology, Child Immunisations, Travel Vaccinations, Family Planning, Health Promotion, Wound Management and Learning Disability Checks.

The ideal candidate would be someone who is motivated and committed to providing high quality care, able to work independently and play an active role in the team.

Though experience in all fields is preferable, training would be discussed for the right candidate.

The salary is based on experience, as well as the usual NHS Pension benefits.  Full DBS check would be required for the successful candidate.

Outer Exeter PCN Clinical Pharmacist

We are looking to recruit 2 WTE self-motivated/enthusiastic Clinical Pharmacists Band 7/8a depending on experience to work within our PCN. As an experienced Pharmacist you will work with the 5 Practices of the Outer Exeter PCN alongside GPs and Nurses. You will report to the Clinical Director who will be responsible for supporting your ongoing professional development. You will provide support to Practice staff to respond to prescribing and medicines queries, medicines reconciliation on transfer of care and long term conditions management. You will carry out patient medication reviews, support medicines safety systems, QI and QOF processes. You will be supported by the Secondary Care Senior Pharmacists who will help you build a network of Pharmacists in Primary/Secondary Care as part of the drive to improve integration between both. You will have good communication skills and work collaboratively with the whole Team to meet the needs of patients, support the delivery of policy and procedures and adapt to the requirements of patients, as needs change. You should be an independent prescriber or expect to work towards this qualification. The final working pattern will be discussed with successful candidates.

Practice Nurse

  • We are looking for an experienced nurse with strong clinical skills and high standards to join our friendly team of nurses.

  • 20-25 hours per week

  • You will have at least 2 years’ experience of working in general practice

  • Our aim is to provide and develop the highest standards of continuing care to our patients, in the context of their families and community by the promotion of good health, prevention of illness and management of disease processes

We have a Nurse Manager and 12 practice nurses supported by an Assistant practitioner and 7 HCA's. We are supported by 2 nurse admin assistants. Although we are a large team we all work closely together and support each other.

We provide nurse cover on 2 sites within Torbay and are part of a wider PCN of 3 Practices. We also have the Extended Access based at our Paignton site and host other services including Lower Limb Service, Midwives, Social Prescribers and Pharmacists.

We also support student nurses within the surgery working closely with the University of Plymouth. A preceptorship is offered via the local training hub and we currently have 3 nurses at different stages on this.

The nurse team are looking for someone who will fit in to our team so will need a good sense of humour and be a team player.
