You will be working over our two sites at Chelston and Barton. We are interested in individuals who have advanced clinical skills and prescribing qualification. General practice experience would be beneficial. You will be expected to help provide the urgent on the day team service for our patients'. This involves minor ailments but an understanding of long term conditions would be beneficial.

This may be by face to face consultation, telephone consultation or home visit. Senior clinician support for this role is constantly available whenever needed but it is expected that you will have the skills to work independently.

Torbay Care Home Visiting Service (TCHVS) is a collaborative multi-disciplinary service providing acute and proactive Primary Care to our large Care Home population in Torbay. 

We deliver four main workstreams on behalf of the 10 Torbay GP practices:

  • An acute medical service to 34 homes

  • The Enhanced Health in Care Homes (EHCH) PCN DES to 84 care homes, incorporating weekly home rounds, medication reviews and care planning

  • A weekly Multidisciplinary Team meeting

  • Medical support for our Intermediate Care Team

We are now looking to expand our team with an enthusiastic (Advanced) Nurse Practitioner or Primary Care Paramedic with an interest in elderly care and multidisciplinary team working.  You will be working flexibly across all four workstreams, but with a main focus on the acute visiting side of the service. 

You will be working from our TCHVS hub in Paignton, together with our GP, pharmacy and operational teams.  You will work closely with our Care Home colleagues- using telephone triage, video consults and face-to-face visiting to assess patients.  You will need excellent communication skills and have experience managing acute and chronic illnesses, frailty and dementia.  Practice nurse experience including chronic disease management and wound care would also be beneficial.

You will be able to work one-to-one with a GP and have pharmacy and administrative support.   You will be encouraged to attend and participate in our weekly MDT meetings with colleagues from across primary care, community teams and secondary care.  We actively promote development within the team to further your skills and expertise, particularly in relation to prescribing, frailty, dementia care, end of life care and advance care planning.

  • Work with the DTH clinical leadership team to support the identification and delivery of DTH training and educational outputs at a locality level

  • Developing a framework for multi-professional learning in the PCNs in their locality and co-ordinating the delivery of this;

  • Working with the DTH team to introduce and support Clinical Supervision for AHPs working in ARRs roles in Primary Care;

  • Coordinate and trial new models of delivering GP training, and when appropriate, dovetail this with multi-professional learning, within PCNs

  • Working with the Devon Training Hub Team to support the scoping of placement capacity expansion within their PCNs; and

  • Support clinical and non-clinical PCN education leads in their workstreams.

 

These posts are available to GPs at any stage in their career who are or have been GP Trainers. We hope they will start with an introductory session at the beginning of September. We are looking to recruit one fellow for each locality area (North, South, East and West).

We are looking for a dynamic Acute Primary Care Practitioner to join our multi-disciplinary team and broaden our clinical skill-mix.
